PURPOSE

This form authorizes the use of photographs, video recordings, and related materials featuring the individual named above for Tabernacle of Faith Church ministry, outreach, and promotional purposes.

AUTHORIZATION

I acknowledge and agree to the following:

  I understand that these materials may be used in print, digital, or online media, including church bulletins, websites, livestreams, social media, advertising, and other outreach platforms.

  I understand that participation is voluntary, that no compensation will be provided, and that all materials will remain the property of Tabernacle of Faith Church.

  
  

DURATION AND REVOCATION

This authorization remains in effect indefinitely unless revoked in writing.
Revocation must be submitted to Tabernacle of Faith Church, Attn: Media Ministry, and will apply only to materials produced after the church receives written notice.

RELEASE AND WAIVER

I release and hold harmless Tabernacle of Faith Church, its pastors, officers, employees, members, and volunteers from any and all claims, demands, or causes of action arising out of the use of my (or my child’s) image, likeness, or recorded materials.
